In the article by Perkins et al, “Cardiac Arrest and Cardiopulmonary Resuscitation Outcome Reports: Update of the Utstein Resuscitation Registry Templates for Out-of-Hospital Cardiac Arrest: A Statement for Healthcare Professionals From a Task Force of the International Liaison Committee on Resuscitation (American Heart Association, European Resuscitation Council, Australian and New Zealand Council on Resuscitation, Heart and Stroke Foundation of Canada, InterAmerican Heart Foundation, Resuscitation Council of Southern Africa, Resuscitation Council of Asia); and the American Heart Association Emergency Cardiovascular Care Committee and the Council on Cardiopulmonary, Critical Care, Perioperative and Resuscitation,” which published ahead of print November 11, 2014, and appears in the September 29, 2015, issue of the journal (Circulation. 2015;132:1286-1300. DOI: 10.1161/CIR.0000000000000144), several corrections have been made to the print version.

On page 1286, in the footnotes, 2 paragraphs about online-only data supplements have been added.

On page 1289, Figure 1 has been replaced with a new image. In the first row (“CORE”), fourth column (“Process”), third line, “Target temp control” has been changed to read, “Target temp management.”

On pages 1289 through 1292, in Table 1, headings to distinguish the various sections were inadvertently omitted in the publish-ahead-of-print version. The following headings have been added:

On pages 1289 through 1292, in Table 1, several corrections have been made:
On page 1289, in the fourth row, third column, first line, “DNAR” has been updated to read, “Total number of cases, number with DNACPR, number considered futile, number with signs of circulation, number unknown.”
On page 1289, in the fifth row, second column, first line, “service” has been deleted after “EMS” to read, “A description of the organizational structure of the EMS being provided.”
On page 1290, in the fourth row, third column, second line, “/not recorded” has been added to read, “Bystander witnessed/EMS witnessed/unwitnessed/unknown/not recorded.”
On page 1290, in the fifth row, third column, fourth line, “unspecified” has been deleted to read, “Home/residence; industrial/workplace; sports/recreation event; street/highway; public building; assisted living/nursing home; educational institution; other; unknown/not recorded.”
On page 1290, in the seventh row, third column, second line, “AED nonshockable/AED shockable/” has been added before “unknown/not recorded” to read, “VF/pulseless VT/PEA/asystole/bradycardia/AED nonshockable/AED shockable/unknown/not recorded.”
On page 1290, in the eighth row, second column, “Pathogenesis/cause is reported as medical (presumed cardiac or unknown, other medical causes); traumatic cause; drug overdose; drowning; electrocution; asphyxial (external cause)” has been deleted and replaced with “The most likely primary cause of the cardiac arrest.”
On page 1290, in the eighth row, second column, sixth and seventh lines, the sentence “Medical (presumed cardiac or unknown, other medical causes): Includes cases in which the cause of the cardiac arrest is presumed to be cardiac, other medical cause (eg, anaphylaxis, asthma, GI bleed), and in which there is no obvious cause of the cardiac arrest” has been updated to read, “Medical: Includes cases in which the cause….”
On page 1290, in the eighth row, third column, first line, “Medical: traumatic cause/” has been changed to read, “Medical/traumatic cause/.”
On page 1290, in the eighth row, third column, first and second lines, “(presumed cardiac or unknown, other medical causes)” has been deleted.
On page 1290, in the eighth row, third column, in the third line, “(external cause)” has been deleted.
On page 1290, in the eighth row, the third column, at the end of the third line, “(Note this variable does not include the unknown option as unknown causes should be assigned as medical causes)” has been added to read, “Medical/traumatic cause/drug overdose/drowning/electrocution/asphyxial/not recorded. (Note this variable does not include the unknown option as unknown causes should be assigned as medical causes.)”
On page 1291, in the sixth row, first and second columns, “TTC” has been changed to “TTM.”
On page 1291, in the sixth row, third column, “TTC” has been changed to “TTM.”
On page 1292, in the second row, second column, the sentence read, “Was the patient’s primary transfer to a healthcare facility able to perform all forms of periarrest and postarrest care and allocated this role by the area of administration?” It has been changed to read, “Was the patient’s primary transfer to a specialist healthcare facility (able to perform all forms of periarrest and postarrest care and allocated this role by the area of administration) or nonspecialist center?”
On page 1292, in the table legend, 2 sentences were added at the end of the first paragraph. The paragraph now reads, “Data definitions have been categorized as core and supplemental. Data definitions have mostly been updated. Registries and researchers may wish to check against their current definitions. The unknown category should be assigned when the variable is recorded as unknown. This differs from the not recorded category, which is reserved for where data are missing or the system does not collect those data.”
On page 1292, in the table legend, the following definitions have been added or changed:
– mm:ss, minutes and seconds”
– “SF-12, 12-Item Short Form Health Survey”
– “TTC, targeted temperature control;” has been changed to “TTM, targeted temperature management”

On page 1294, Figure 2 was replaced with a new image. The corrections made to it were:
On the left side of the figure, in the second box (“Cardiac Arrests Attended”), the second line, a semicolon after “Total” was removed. The line now reads, “Total Number of Cases.”
On the right side of the figure, in the second box (“Dispatcher ID…”), in the left top panel, the heading “Dispatcher ID Cardiac arrest” has been changed to read, “Dispatcher ID CA.”
In the box at the bottom (“Patient Outcomes Reporting Population”), in the row “Shockable bystander witnessed,” an asterisk has been added to read, “Shockable bystander witnessed*.”
In the figure legend, the following changes have been made:
– The definition “CA, cardiac arrest” has been added.
– “Fav” has been defined as “favorable.”
– The text “*Utstein comparator group (system efficacy)” has been added.

On page 1295, first column, second paragraph, third sentence, “also available as Online Data Supplement 2” was added after “Figure 2.”

On page 1295, first column, second paragraph, 11th sentence, the text “The 2014 template allows reporting of the witnessed cardiac arrest, who received bystander CPR and had a first recorded rhythm that was shockable (which is recommended as a comparator of system efficacy), and all EMS-treated cardiac arrests (which is recommended for system effectiveness comparisons)” has been updated to read, “The 2014 template allows reporting of the bystander witnessed cardiac arrest who had a first recorded rhythm that was shockable. This is recommended as the Utstein comparator of system efficacy. All EMS-treated cardiac arrests are recommended for system effectiveness comparisons.”
